IS THE INSURANCE PREMIUM?

How
many units must I be enrolled IN to BE eligible to purchase this insurance?
You must be enrolled in a minimum of 6 units and you must actively attend
classes on campus for at least the first 31 days after the date for which
coverage is purchased. Home Study,
correspondence, internet, or TV courses do not qualify.
Can
this insurance be paid on a month-to-month basis or in installments?
Month to month is not an available option, however, you may purchase 3,
6, or 12 months.
What
methods of payment are available for this insurance?
The preferred payment method is by personal check, corporate check, money
order and/or cashier check. In the event
you have no access to such method of payment, you may pay by Visa or Master
credit cards, but you may not pay over the telephone. We are not responsible if you enclose
cash in the envelope. You may also
enroll on-line by clicking here.
When
does my insurance become effective?
If paying by check, insurance becomes effective on the date the
application is received in our office (subject to negotiation of the
check). If paying by credit card, the
effective date is the date of credit card approval or your requested effective date,
whichever is later.
If
I leave school or transfer to another school can I still keep this insurance?
Yes. Coverage ends the last day of
the term chosen and for which premium has been paid, regardless if you transfer
or leave school. Please refer to the Eligibility on page 1 of the brochure.
Where
can I find an application to purchase this insurance?

in the health or student center at your school campus.

What
TYPE of insurance is this and what does it cover?
This is a Health and Accident insurance plan. The insurance benefits are described in the

DOES
THIS INSURANCE HAVE DENTAL OR VISION BENEFITS?
Dental and
Vision benefits are not covered under this plan; however, you may purchase a
Dental and Vision Discount Plan from our website by clicking here.
Can I get my money back?
There are no refunds made unless the student enters military service and
a pro-rata refund of whole unused months will be made.

I have to go to a specific doctor or do you have a list of participating
doctors?
